  12. Where are you uploading the file?
    I mean what is the Webserver you are using?
    If IIS is the webserver ,go to website properties and under documents section
    make sure Enable Default document is checked and mention the page name (along with extension) which you want to make as start page.
    I am sure there will be some similar setting in other Webservers too
